OpenAI has released new internal data claiming that a small fraction of ChatGPT users show signs of psychosis, mania, or suicidal intent during weekly interactions with the model. But internal definitions, detection thresholds, and AI-driven interpretation suggest the numbers may not represent what most would consider genuine clinical signals.
In the report, OpenAI estimated that about 0.07% of ChatGPT users — roughly 560,000 people per week out of its 800 million active users — display what it described as “possible signs of mental health emergencies related to psychosis or mania.” The company also said that 0.15% of users, more than one million per week, express some form of “suicidal planning or intent.”
The figures mark the first time OpenAI has quantified mental health-related patterns among its users. The company said it collaborated with more than 170 mental health professionals to refine a system that recognizes “distress signals” and routes users toward supportive resources.
Yet some experts and observers note that the figures may be dramatically overstated due to the model’s highly conservative safety filters and linguistic patterning. ChatGPT often misinterprets figurative speech, artistic writing, or philosophical discussions as warning signs, leading to false positives that can skew aggregate data.

I appreciate that there is an effort to identify mental issues in the AI, but I don’t understand how an AI will ever be able to do this with great accuracy. People “say” all sorts of strange things “because they’re venting, dramatizing, or even testing how the AI will respond.”
I don’t think that these efforts should be stopped but if the numbers don’t match reality that needs to be taken into effect.
